QDMerge is a Perl program that Merges text from a source document with a template to generate a unique output file that follows a specific design. (Fully Buzzword Compliant)
It's currently on version 0.4 (it works, and it supports even more cool features). There's a development version out right now, named 0.54. It's changed somewhat, so you'll have to do a little work to get it to work. There's a world of difference internally between the two versions, giving us much more flexibility at the cost of a little more upfront work.
It's also Free Software, which doesn't only mean that you don't have to pay me to use it. No, I'm not crazy. "Free" is like the French word libre, not gratis, even though I'm not charging for it.
QDMerge is also extensible, which means that with some clever programming (some actually done by me), it's not limited to manual offline HTML generation. For example, this version supports a configuration file which lets you specify multiple files and templates to process. Maybe I should mix metaphors and call it a recipe (since bits did come from the Perl Cookbook).
